Adult Outpatient ServicesProgram DescriptionServing the Will County area, the Adult Outpatient Services Program provides community-based mental health outpatient services for those individuals who are seriously/chronically mentally ill or who may be at risk of becoming seriously/chronically mentally ill. Priority for receiving services is reserved for those individuals being discharged or deflected from a local or State operated facility (SOF) inpatient psychiatric unit.The program incorporates the following values and principles:client-focused and community-basedaim to empower person served, to be family-focused when appropriate, to be provided in a manner which is ethnically an culturally appropriateprovided on and off sitebuilds on the strengths of the person served and shall seek to assist the person served in functioning in the community to the fullest extent possiblecoordinated with other community care providers and emphasizes continuity of careseeks to protect the rights of the person served and to assist in advocating for his/her rights, andis in accordance with current clinical knowledge and accepted standards of practice.Persons served are assigned to multidisciplinary treatment teams who have the collective responsibility and accountability, in conjunction with the active participation of the person served, for the assessment, development and implementation of an individual treatment plan and the provision of services on and off site.Population ServedAdults 18 and older who meet the following criteria:Individuals who have had multiple, lengthy or frequent admissions to a SOF and/or community inpatient psychiatric unit.Individuals who may be at imminent risk of hospitalization, who may be more appropriate for less restrictive services that may be available in the community.Individuals who have a serious and /or chronic mental illness and who need ongoing services to remain in the community, with a particular emphasis on those being discharged or deflected from a SOF.Individuals who are Medicaid eligible to receive services pursuant to DHS/OMH Medicaid Rule 132.Program GoalsTo provide community-based mental health psychiatric treatment services including, psychiatric evaluation, medication services (prescriptions, administration, monitoring and self-medication training), individual and group counseling and psychotherapy, client-centered consultation, case management, and crisis prevention and intervention:Program ServicesInitial service needs evaluationMental Health Assessment, comprising a Social History and Clinical AssessmentPsychiatric evaluation, when indicatedPsychological Assessment, when indicated and ordered by a physicianIndividual Treatment Plan (ITP) developed, implemented and modified with the active participation of the person served.Psychiatric Treatment Servicesindividual counseling and/or psychotherapyfamily/couples counseling and/or psychotherapygroup counseling and/or psychotherapypsychotropic medicationmedication administration, monitoring and self-medication trainingClient-centered Servicesconsultation with other providersreferral, follow-up and coordination of services with other providers such as Office of Rehabilitation Services, Public Aid, Social Security Administration, Substance Abuse Programs, Support Groups and the Will County Community Health Center for medical treatmentmental health case management servicescrisis intervention servicesProgram StaffingAdult Outpatient Services are provided by a staff of professionals including:Licensed Clinical Professional CounselorsLicensed Clinical Social WorkersRegistered NursesBoard Certified or Eligible Consulting PsychiatristsBachelor Level Case Managers.In addition, there are 6 Board Certified or Eligible Consulting Psychiatrists.

DFSS Community Service Centers help individuals and families in need access a wide range of resources from emergency shelter, food and clothing to domestic violence assistance, job training/placement and prisoner re-entry. Clients can get information about rental, utility and other financial assistance programs. The facilities serve as Warming and Cooling Centers during periods of extreme weather.
